GrafTech partners with Anthem to offer two medical plans to eligible employees and their eligible dependents. The Anthem Consumer Plan and Anthem PPO Plan have unique components that may be attractive to your situation. It’s important that you understand the differences and select the plan that best meets your health care needs and your budget.
Eligibility
GrafTech employees working 30 or more hours per week are eligible for Medical benefits. Eligible dependents can also be covered.
Anthem Consumer Plan
Anthem Consumer Plan Puts You in Charge
The Anthem Consumer Plan can help you manage your healthcare expenses. If you participate in the Anthem Consumer Plan, you:
- Enjoy the lowest monthly premiums of the two plan options
- Can open a Health Savings Account to save money for medical expenses tax-free
- May be eligible for GrafTech’s contribution to your Health Savings Account
- Cannot be covered by any other medical plan such as Medicare or a spouse’s medical plan
Anthem PPO
Anthem PPO Plan Offers Predictability
The Anthem PPO plan covers many network physician’s services after nominal copayments which will accumulate to your out-of-pocket maximum. In addition, if you participate in the Anthem PPO Plan, you:
- Will have the lowest deductible
- Will pay the highest premiums
- Will have the lowest out-of-pocket limits
- May also be covered by another medical plan such as Medicare or a spouse’s medical plan
REMINDER: A $200/month Spousal Premium applies to the Anthem PPO Plan. It is a requirement for you to certify that your spouse does not have other coverage available to him/her. If other coverage is available, and you choose to cover your spouse on the GrafTech plan, the spousal premium of $200 will apply in addition to the amount you pay for medical coverage. The charge will be waived if enrolled in the Anthem Consumer Plan.
